The application process:

1.  Fill out the following online pre-registration form. Read carefully and complete all required fields.

2.  Fill out the attached application packet completely. (You will be directed to the packet when you click Submit at the bottom of this pre-registration form.)

NOTE: The application packet is a PDF that contains a Permission Slip, Medical Treatment form and Student Release Authorization form to be completed and signed by the student and the Parent/Guardian.

3. Send completed and signed forms, via U.S. mail, to:

NOTE:  We are ONLY accepting ONLINE payments in the form of credit cards.
